Case Management

Services Offered by Washington County Children's Program

At the Washington County Children’s Program, we like to say "we don't 'manage cases,' we work with families." Families are not cases, they are unique groups of people with unique wishes, hopes, and dreams. All of our work is family-centered and strengths-based. That means we look first for what is going well, and build on those strengths to help meet the family’s needs.

A WCCP case manager can do the following for a family with a child ages birth to 21 who has a diagnosed special need or, for ages birth to six, is "at risk" for developing special needs:

  • Meet with families to discuss strengths and needs in order to support your child in home and community.
  • Attend team meetings to support your family’s needs, provide information, and advocate for services.
  • Help access funds or resources that will benefit your child.
  • Help develop family support systems through workshops, parent groups, and newsletters.
  • Assist your child and family through periods of transition - times that are most difficult for a child with special needs.
  • Provide free access to a library of resources - books, DVD’s, audiotapes, and national newsletters.
  • Smooth the way between families, schools, and providers to promote consistency for the child.

A case manager can meet with you in your home, at our offices, and/or at your child’s school, and is also available by phone during business hours.

Targeted Case Management

Targeted Case Management is available to families in Washington County with a child birth to 21 with a diagnosis of autism, mental retardation, or pervasive development delay (PDD). We also provide case management for children with a diagnosis of autism in Hancock County. Families may self-refer for this service by calling our office.

Early Intervention Case Management

Early Intervention Case Management is available to any family in Washington County who has a child at with, or at risk of, a developmental delay. No medical diagnosis is necessary. One of our caring professionals will meet with you to assess your child’s needs. This service can be self-referred by you, the parent. Medical professionals can also make referrals directly to WCCP. Please call our office for more information.

First Point of Contact
